About Jade Kristiansen

As a Care Coordinator, my goal is to provide a supportive and welcoming environment where you can comfortably share your concerns and needs. I will guide you through our available services, and provide support throughout the intake and placement process until you are connected with the providers best suited to your needs. I am especially passionate about working with children and families and focused on at-risk youth specifically, and the challenges they face during my undergrad.
