We’re excited to help you begin your journey to mental health wellness. Please use the following resources to prepare for your first appointment, find your nearest Elite DNA Behavioral Health office or access our Patient Portal.

We’re so glad you’re here!

When you are a new patient, your first appointment is an initial evaluation with a qualified coordinator or clinician. During your initial appointment, you will review your medical history and concerns with a Care Coordinator, a therapist or a nurse practitioner. This initial appointment is essential to ensuring you receive the right care.

After your initial appointment, you will have your next steps in your care plan, whether that be to see a psychiatrist, a therapist, or both. Once you see your provider(s) we will create a comprehensive care plan in partnership with you. We will meet you where you are – both in your mental health journey and with in-person and telehealth appointments.


Preparing For Your First Appointment

Elite DNA uses a system called Phreesia to help streamline your check-in process and make it easier to access the care you need. Prior to your first appointment with your provider, you will receive a link to your email address and/or mobile device to complete your patient paperwork. This step can be completed via desktop or on your mobile phone.  

Please make sure to bring a list of any medications that you are taking currently.  


Exploring The Benefits of the Patient Portal

After your first appointment, you will be invited to join Elite DNA’s Patient Portal, which allows you to have your health information right at your fingertips. You can send secure messages to your Provider, request prescription refills, find your lab and test results, and much more. Plus, the Patient Portal is available 24 hours a day, 7 days a week and 365 days a year! We encourage our patients to sign up for the Patient Portal and utilize it throughout the year to quickly and efficiently manage their healthcare.
