What is Care Coordination?

Care Coordination, also referred to as an intake appointment, is a process that Elite DNA Behavioral Health pioneered, that allows us to assess your needs and ensure we pair you with the right services and the providers that match your needs. This appointment can be taken over telehealth or in-person (pending Care Coordinator availability). Studies show that when care is coordinated in this way, our patients enjoy improved outcomes in wellness.

What should I expect for my first appointment?

As a new patient or a patient who has not been seen by us in a year or longer, you will see a Care Coordinator for your intake appointment. Your Care Coordinator will take in your medical history, review your needs with you, and help schedule you for therapy, psychiatry or another service. Your next appointment will be an evaluation by a provider.

What is the difference between an Intake Appointment and a Therapy/Psychiatry evaluation?

The purpose of an Intake Appointment is to evaluate your high-level behavioral and mental health needs and get you scheduled for your next appointment, which is an evaluation with a mental health professional who can begin building a treatment plan with you.

Where is Psychiatry and Therapy offered?

Both services are offered widely across most offices.

Where is Speech, Occupational Therapy and Speech offered?

We offer Occupational Therapy, Speech and Applied Behavior Analysis at our Fort Myers Plantation office . Our Naples office also offers Occupational Therapy.

Do I need a referral to schedule an appointment?

Some insurance plans may require you to have a referral, though most do not.

How do I find a provider who is right for me?

During your intake appointment, a Care Coordinator will assist you in selecting a provider that fits your needs.

Can I ask for a female or BIPOC provider to ensure I’m comfortable?

Yes, as the patient you are entitled to request a preference regarding your provider. We understand that there are shared cultural experiences that can be helpful in a provider. Simply ask your Care Coordinator or scheduler. It’s 100% acceptable to make these requests.

What do I need to do before my appointment?

Once your appointment is scheduled, you will get a notice from us in the Patient Portal to complete your paperwork. If you’re a new patient, you’ll start with your Care Coordination appointment paperwork, have your initial intake appointment and then get scheduled with your provider from there. If you’re an established patient, you will still get a request from us to complete paperwork.

How do I dial into my Telehealth appointment?

The morning of your virtual appointment with your Care Coordinator or Provider, you’ll receive a text message or email reminder (depending upon what you have opted into) that your appointment is taking place that day, with a link to zoom along with the meeting ID and passcode. The meeting ID is unique to each patient appointment. The passcode is always DNA. If you have an appointment and you already have your meeting ID and passcode, you can visit our Zoom page and enter the information here to sit in the virtual waiting room.

Do you offer transportation?

At this time we do not offer transportation. However, we offer Telehealth services to help with accessibility.

How do I cancel or reschedule my appointment?

These requests can be made through the patient portal. You may cancel your next appointment completely through the portal, and then can request to reschedule an appointment and one of our schedulers/office staff will contact you to get you rescheduled.

What insurances do you accept?

We accept a large variety of insurance plans, nearly every plan in Florida including Medicaid, Medicare and Blue Cross Blue Shield. Visit our insurance page for details .

What fees am I responsible for?

You are financially responsible for the cost of the mental health services provided to you or your dependent(s) and for any portion of the fees not reimbursed or covered by your health insurance, including late cancellations and missed appointments.

Do I need my insurance card?

Yes, please bring your insurance card to each of your appointments. A copy of your up-to-date and active insurance card is required for your appointment. If you do not have a copy of your insurance card, a $50 hold will be placed prior to your appointment. A copy of your insurance card can be uploaded by our staff or through the patient portal .

Do I pay a copay for virtual/telehealth appointments?

Copays are collected prior to your appointment. In-person patients can pay copay at reception, and virtual patients are required to have a credit card on file to bill your copay. If you have insurance, and it is accepted by Elite DNA, your insurance will be billed for the appointment. Any copays required by your insurance plan will be billed at time of service. We recommend reviewing your explantation of benefits from your insurance company so that you know what to expect. For patients with no insurance, please inquire with your office for sliding scale and self-pay options.

Do I have to put a card on file for my copays?

Virtual patients are required to put a credit card on file. This can be done via phone or through our patient portal .

How do I pay my copay for my telehealth/virtual appointment?

Virtual patients are required to have a credit card on file so we can charge your card after your appointment. This can be done via phone or through our patient portal . If your credit card is not on file prior to your appointment you may experience a cancellation of your appointment.

Do I pay a copay for my first intake appointment?

Yes, you will pay a copay as your insurance plan directs for your intake appointment with a Care Coordinator.
