How Billing Works
Elite DNA accepts most insurance plans in Florida, including Medicaid, Medicare, VA Benefits and commercial insurances like United Healthcare and Blue Cross Blue Shield. Please see all insurances here.
Bring Your Insurance Card
We ask that you provide your insurance card at each visit to allow us to verify your benefits. If you do not bring your insurance card with you, there will be a $50 hold to be paid at time of service. After the hold you may provide your insurance card to our front desk at your next appointment or upload it into our patient portal.
Copays
Copays are collected at time of service. For in-person appointments, payment will be accepted at the front desk before your appointment begins. Virtual patients will be required to place a credit or debit card on file. If there is not a card on file for your virtual appointment, the appointment will be canceled 90 minutes prior to your appointment time. Self-pay will also be collected at the time of service.
Self-Pay
Elite DNA offers the ability to do self-pay without insurance if that is the preferred method. Payment will be collected at time of service for self-pay.
