Prior to your next appointment, be sure to check for a text or email with an invitation to check in for your appointment.

Elite DNA Behavioral Health has partnered with Phreesia to enhance your check-in experience. Say goodbye to lengthy office paperwork, and say hello to flexible and quick appointment check-in via desktop or mobile. 

Here’s What to Expect:

• 3 days before your appointment, you’ll receive a text or email with a secure link to check in. If you are a new patient, you will receive additional intake questions. 
• Follow the instructions for providing the information we need for your upcoming appointment.
• You’ll also have the chance to update your medical information, pay your co-pay, and sign any necessary forms.
• 30 minutes prior to your telehealth appointment, you will receive a link to fill out any remaining registration questions. You will then be prompted to join your provider’s waiting room. Click the button to join your provider’s waiting room.
